Georgia
Any permit load requires a Certificate of Liability that consist of the following amounts of public liability: $300,000 each accident; $100,000 each person injured; total property damage, $50,000; public property damage, $50,000. The certificate must show the Department of Transportation as the certificate holder and must remain of file for the duration of the permit. This requirement may be found in the Official Rules of the State of Georgia, Department of Transportation, 672-2.03(d).

Louisiana
Louisiana requires that a certificate of insurance, which insures any damage to state property, must be posted at the Truck Permit Office before any permit will be issued. The amount of coverage required is a minimum of $100,000. Automobile liability coverage is required for truck and trailer; Off-road units require general liability coverage.
The Acord certificate must be attached in a PDF and should show the effective date, expiration date, property damage coverage and the insurance contact (name and phone number). Permit service can upload the certificate to the Louisiana permit system.
Mississippi
A Certificate of insurance for $500.000 combined single limits and a valid USDOT number must be on file with the Mississippi Department of Transportation Permit Office. More information contact.

Tennessee
Companies and Corporations must submit proof of insurance on the ACORD form. An example of this form may be found on our website under Resources.
Private individuals must provide proof of insurance, but the ACORD form is not required.
Any person, firm, company, corporation, or other who undertakes the movement of any overweight and/or over dimensional article and/or commodity on the highways of the State of Tennessee shall hold the State of Tennessee, its officers, and employees, harmless from any claims for damages resulting from the exercise of any of the privileges granted under the special permit so issued for such overweight and/or over dimensional movement, and to this end, shall carry liability insurance with an insurer, acceptable to the TDOT Permit Office, and shall furnish a certificate of insurance to the TDOT Permit Office, in the amount of not less than three hundred thousand dollars ($300,000) for each claimant injured and one million dollars ($1,000,000) per occurrence.
Additionally, the certificate of insurance (COI) shall provide that the insurer shall give, to the TDOT Permit Office, written notice of intention to terminate said required insurance by certified mail, said termination to become effective thirty (30) days after receipt of said notice from the insurer by the TDOT Permits Office.

There are many local municipalities and counties that require permits in addition to what is issued by the states. It is advisable that you check with those local authorities when you are traveling on routes that are not authorized on you permit. Some of these local jurisdictions also require insurance on file in order to obtain the necessary permits required to travel on their roads.
